Staten Island UH Brain Injury Waiver Program

The Home and Community-Based Services Medicaid Waiver for Individuals with Brain Injury (HCBS/TBI) is a program jointly sponsored by the New York State Department of Social Services (DSS) and the Department of Health (DOH).

Eligible clients may be assisted with discharge from nursing facility to the community and funded for housing. Services provided assist the individual to increase independence, self reliance, productivity and inclusion within the community.

Individuals eligible are between the ages of 18 and 64 with a Traumatic Brain Injury Diagnosis who choose to receive services through this waiver rather than be a resident of a nursing facility.
